As Telehealth Care Team Lead, you will serve as the clinical anchor for a remote, interdisciplinary team managing high-acuity Medicaid members across Arizona. You'll own clinical decision-making, team performance, and care coordination strategy — directly influencing patient outcomes, total cost of care, and the scalability of our care model.
You'll work at the intersection of complex case management, telephonic triage, and cross-functional care coordination — leveraging Allscripts Care Management, Allscripts ED, and Paragon Inpatient Physical Rehab to keep the full picture of each member's health in view. You'll partner with behavioral health specialists, social workers, care coordinators, and external providers to build care plans that actually stick.
